Fee-For-Service Clients
Social Security's permits organizational Fee-For-Service Representative Payees to provide services to clients that reside within 75 miles of the agency's office. This means that Finance for People, Fee-For-Service can serve most residents of Mendocino, Sonoma, Humboldt and Del-Norte Counties. We advise that you contact our office prior to filling out an application to discuss our eligibility to serve as your Representative Payee.
Please note, that if the client does not currently have a payee, a SSA-787 Physician Statement will need to be completed as required by Social Security.
The Application Process
Application packets can be completed by prospective clients, case workers, support staff, or a trusted confidant. Please answer the questions on this application to the best of your ability.
Once the application has been completed, please return all pages to our agency for review. We will then submit your application to Social Security. Social Security may contact you regarding our application to serve as your Representative Payee. Please be prepared to speak with an agent and answer a few questions.
Social Security will need to determine that you need a payee, not just that you wish to have a payee. If you are currently receiving your Social Security benefit directly, a SSA-787 Physician Statement will need to be completed. Additionally, they will need to assess that our agency is best suited to serve as your payee and that you do not have any family members that are willing and fit to serve as your payee. Our application includes form SSA-795 Statement for your to express why you are requesting our agency serve as your representative payee.
Processing times may vary and can be anywhere from three to six months. Please understand that Finance for People is not Social Security, does not have direct access to your Social Security information, and does not have control over how quickly your application is processed.
Finance for People will follow up with you and your support team as updates and Social Security notices are received. If you would like Finance for People to access information regarding your Social Security record prior to us becoming your payee, you may include form SSA- 3288 Consent for Release of Information with your completed application packet.
